Since 2012, Population Health has been a particular focus area for ADG. We have successfully led projects that improve outcomes for high risk and high utilization patients across a number of hospitals and programs.

When a regional hospital partnership was formed to seek HSCRC funding for a state healthcare transformation grant, they partnered with ADG for the technology portion of the grant proposal. Once the grant was secured, the partnership asked ADG to lead the design and implementation of a data warehouse and associated data visualizations. The data warehouse included data from multiple EHRs, Care Management data, and health information exchange data including ADT feeds and C-CDA files. The visualizations led to improved outcomes for patients characterized by multiple chronic conditions and high hospital utilization.

When an Accountable Care Organization (ACO) wanted to automate their ACO reporting process, they turned to ADG to lead the project. ADG architected and populated a data warehouse with data from multiple hospital and ambulatory EHRs and then developing reports for the measures in each ACO functional area. The effort was then expanded to produce Provider Scorecards that gave a real-time view into how ACO practices and providers were performing when compared with each other as well as established benchmarks. The Scorecards included detail level reporting which led to improved ACO metrics as well as improved outcomes for patients.

  • Conversion of legacy EHR data
  • Extending Epic Caboodle to provide claims-based Inpatient and ED utilization metrics
